Preoperative risk factors for difficult laparoscopic cholecystectomy

ABSTRACT
Introduction: the present investigation was carried out, with the main objective of identifying the preoperative risk factors, which influence the performance of a laparoscopic cholecystectomy considered difficult.Methods: a prospective, observational study of cases and controls was carried out over a period of three years in the Laparoscopic Video Surgery Service of the Carlos M. de Céspedes Hospital. The universe was constituted by the 1009 cases of cholecystectomy Laparoscopic video, the independent variables were operationalized and as a dependent variable, difficult laparoscopic video cholecystectomy was considered, and the data was processed using the SPSS version 23.0 statistical package.
Results: a total of 436 cases and 573 controls were studied, with a predominance of females in the studied sample. In the clinical history, abdominal pain was the most important, 93.3% of which were superior to controls, BMI and the associated diseases did not show significant results, acute cholecystitis 20.4% and chronic 8.3% diagnosed by ultrasonography in the cases were much superior to the controls.
Conclusions: abdominal pain was strongly associated as a risk factor for a difficult laparoscopic video cholecystectomy, with statistically significant results in the univariate analysis. At the same time, the ultrasound diagnosis of acute cholecystitis was a risk factor for a difficult laparoscopic video cholecystectomy, with statistically significant results in the multivariate analysis.
