ABSTRACT
A case of a 14-year-old-boy who started with back pain associated to walking disorders is presented. Simple and contrasted studies of the spinal cord were performed, which showed a fracture of the 12 th dorsal spine fracture with medullary engagement. Clinical manifestations, imaging studies such as plain X rays of the spinal cord, myelogram, and CT-Scan with myelogram using an equipment of 64 cuts run at the emergency department were presented. Treatment with steroids was initially prescribed, and before 24 hours of diagnosis of medullar compression, the patient was taken to the operating room where a multidisciplinary team of Orthopedics and Neurosurgeons performed a decompression laminectomy in D11- D12-L1 spaces, a colectomy of D-12, and the tumor resection with Luque instrumentation. The patient had significant improvements and initial symptoms disappeared. We conclude the most likely diagnosis in patients with clinical manifestations of medullar compression without history of trauma should be that of osteolisislession resulting from a tumor for which simple or contrasted imaging studies should be run to diagnose and treat the patient quickly to avoid Neurological sequelae.
