Skin metastasis of ovarian clear cell carcinoma after herpes zoster: an isotopic Wolf response

ABSTRACT
Cutaneous metastases from ovarian origin occur in less than 4% of the patients. A 44-year-old female, with clear cell ovarian carcinoma, manifested a cutaneous viral infection (diagnosed as herpes zoster) that completely resolved with treatment, but then she had persistent lesions at the same location. Histopathology showed clear cells with similar morphology to the primary cancer known invading dermal vessels, which confirmed metastasis. Additionally, pleural metastasis was found, with ominous progression to death. Cutaneous metastases can exotically appear as an isotopic Wolf response after a previous dermatosis, especially in viral infections. This phenomenon has been reported in cutaneous metastasis, and only one case of papillary cystoadenocarcinoma of the ovary. This case is the first reported with ovarian clear cell carcinoma. Cutaneous metastases are rare; they represent a worse prognosis at the time of diagnosis. Early diagnosis is essential, in which the role of the dermatologist is significant.
