Discordant results of thyroglobulin and scintigraphy in patients operated of thyroid cancer

ABSTRACT
In medical assistance, we can find discordant cases in the follow-up of patients with differentiated thyroid cancer (DTC), either patients with low thyroglobulin (Tg) levels, with positive scintigraphy or patients with high Tg levels and negative scintigraphy. The aim of the study was to evaluate the behavior of thyroglobulin levels and scintigraphy in patients operated of DTC.
Material and methods: A retrospective study was carried out in 148 patients operated of DTC. The follow-up included the performance of iodine-131 scintigraphy and the determination of thyroglobulin, thyroglobulin antibodies and thyroid stimulating hormone. The Pearson correlation test was used to study the association between the variables.
Results: The 52.7% of the cases corresponded to discordant results: 5.1% with low serum levels of Tg and positive scintigraphy and 94.9% high Tg levels and negative scintigraphy.
Conclusions: Scintigraphy and Tg studies did not correlate in 78 patients studied, 52.7%. The highest number of discordant cases corresponded to values of positive Tg and negative scintigraphy. Knowing the possible causes of these discrepancies, as well as the limitations of each diagnostic procedure, will help a more effective follow-up of these patients.
