Amortiguated grafts for nasal dorsum rhinoplasty

ABSTRACT
Background: The deformity of the nasal dorsum represents a significant challenge for the facial plastic surgeon.Objective: To offer a new technique for the use of cartilaginous grafts applied to the hypoprojected nose that optimizes the surgical results, gaining more height.
Material and Method: A retrospective study was done at the Instituto de Alta Especialidad en ORL located in the city of Villahermosa, Tabasco, Mexico, from January 2013 to December 2017, with cases with the diagnosis of hypoprojected dorsum and nasal tip, with preoperative and postoperative follow up at one month and one year. We performed the cupuloplasty technique and a plate of septal cartilage amortiguated with an elliptical cartilage formation, which was taken from the cephalic border of the lateral inferior crura, was placed at the dorsum. Also, a septal cartilage graft amortiguated with an elliptical cartilage formation was placed above the anterior nasal spine and anterior to both medial crura.
Results: A month later this angle was from 17.90° to 22.2° and one year later an average of 20.35° was reached. Regarding to the nasofacial and nasolabial angles, we found a considerable average improvement. In the nasolabial angle there were 27º of increasing at 12 months and in the nasofacial angle there were 7.9º of increasing at 9 months.
Conclusion: This technique increases the nasal angle and thus the height of the tip and the nasal dorsum in a very satisfactory way.
