ABSTRACT
Ventilation with airway pressure release is an intermittent mandatory modality, cycled by time, controlled by pressure, with inverse inspiration / expiration ratio that allows spontaneous breathing throughout the respiratory cycle. The objective of this article is to describe the behavior of oxygenation in ventilated patients with airway pressure release. We present two young patients with severe respiratory distress due to influenza A H3N2. They were ventilated in a volumecontrolled mode. They needed high levels of positive pressure at the end of the expiration without achieving the desired oxygenation goals and with slight CO2 retention. One of them was hemodynamically unstable and therefore required vasoactive support. As a rescue strategy, the use ventilation with release of pressure was decided. Both cases showed improvement in oxygenation with lower levels of FiO2 and pressures in the airway.
