ABSTRACT

Introduction: ovarian tumors are the third group of tumors diagnosed in women. Mature teratoma of the ovary or dermoid cyst is a benign tumor that represents more than 20 % of all neoplasms in that organ but its development as a whole with twin gestation is rare.
Objective: to present the case of a patient with a twin pregnancy who was diagnosed with two giant ovarian cysts, of interest to specialists due to the size reached by the cysts and the favorable evolution of the mother and the neonates.
Case presentation: female patient of 28 years of age, white skin color, and urban origin, with twin pregnancy. An ultrasound was carried out at 30 weeks of gestation and two fetuses of good weight for their gestational age and a giant cyst in the right ovary were detected. Her evolution was followed by ultrasound and at 38 weeks she was operated surgically, finding another cyst of similar dimensions and weight, located in the left annex.
Conclusions: most ovarian cysts are diagnosed incidentally because they are usually asymptomatic. Ultrasonography in the diagnosis makes possible to adequately follow the evolution of pregnancy and the growth of the cysts. However, the surgical finding of the second tumor demonstrated the limitations of this diagnostic technique in a multiple pregnancy. It is advisable to follow the evolution of pregnant women through frequent examinations, with a view to identifying functional alterations.
