Embolización ecodirigida de pseudoaneurismas con trombina humana en pacientes del Servicio de Angiología del Centro Médico Nacional de Occidente del Instituto Mexicano del Seguro Social. Estudio de serie de casos

ABSTRACT
Objective. To show the results at twelve months after the eco-directed embolization of pseudoaneurysms
with human thrombin in patients of the Angiology Service at Centro Médico Nacional de Occidente of
Instituto Mexicano del Seguro Social.
Material and methods. A case series study was conducted to evaluate the results obtained at twelve
months after the eco-directed embolization of pseudoaneurysms with human thrombin. We consider
effective treatment the absence of flow within the pseudoaneurysm sac, with integrity of pulses and distal
flows, evaluated with color Doppler ultrasound.
Results. Seventy patients were included. Using human thrombin, we obtained complete thrombosis of
the cavity and neck of the pseudoaneurysm, except in 11 cases, which had 2 cavities, for which we used
2 injections. There were no serious complications, death secondary to the procedure or adverse effects secondary
to the use of thrombin; however, two patients presented pain immediately after the procedure
and one of them decreased the range of movement, which improved 30 days after the procedure. In the
30-day evaluation, 2 cases of recurrence (2.86%) were obtained, finding presence of flow within the sac,
which were subjected to open surgical repair; the remaining 68 patients reported arterial integrity with
complete occlusion of the pseudoaneurysm at 365 days, obtaining efficacy of 97.14%, comparable with the
world literature.
Conclusions. Eco-directed embolization is an effective, rapid, safe, and economic technique, which is
why we consider it the center of choice to manage this pathology.
