Oculo-orbital lesions in patients with craniofacial trauma.

ABSTRACT
Purpose: To know which ocular and orbital injuries are more frequent in patients with craniofacial injuries seen in a Traumatology Hospital.Material and methods: A retrospective transversal study was made. The files of patients requiring ophthalmologic review after sustaining craniofacial trauma were reviewed (from 1998 to 2001). The main injury was registered since first evaluation.
Results: 2759 eyes with ocular and or orbital injury of 2701 patients with craniofacial trauma were evaluated. 2759 injuries corresponding to 46 pathologies were found. The following lesions were found: Anterior segment injuries were the most frequent with 1301 cases (37.36%), 320 (11.59%) of them had subconjunctival hemorrhage. 419 (15.18%) Ocular adnexa injuries were reported and 196 (7.10%) presented blepharoequimosis. Orbital injuries 353 (12.79%) with 181 (6.56%) complicated fractures. Neuroophthalmologic injuries were found in 325 (11.70%) with 85 (3.08%) traumatic optic neuropathy. Oculomotor injuries 323 (11.70%) with 135 (4.89%) of oculomotor dysfunction. Posterior pole injuries 308 (11.16%) with maculopathy in 96 (3.47%).
Discussion: The spectrum of injuries found in craniofacial trauma patients is wide; anyone of them can be a threat to the visual function.
Conclusions: In every ocular trauma we must suspect hidden lesions even if there is no evidence of external damage because vision loss can result.
