Acute abdomen in a patient with situs inversus

ABSTRACT
Acute abdomen is a clinical syndrome which comprises all recent-onset abdominal pain
with repercussions in the patient’s general state, and requires a rapid and precise diagnosis,
in view of the possible need for urgent surgical treatment. Situs inversus totalis is an
uncommon recessive autosomal genetic condition, characterized by the inverted position of
the thoracic and abdominal organs in relation to the sagittal plane. Appendicitis is the most
common cause of emergency surgery worldwide, with a prevalence of 7-8%. We present
the case of a male patient, age 36 years, with 6 hours’ evolution of periumbilical abdominal
pain, which was localized in the left iliac fossa, with tachycardia, fever, and leukocytosis;
in computed tomography, we observed situs inversus totalis, appendix on the left, post-ileal
localization, increase in thickness, thickening of its walls, heterogeneous enhancement,
and submucosal edema.
