Ultrasound-guided infraclavicular brachial plexus blockade comparing two approaches: coracoid versus costoclavicular, evaluation of scientific evidence

ABSTRACT
The brachial plexus block for distal upper limb surgery can be achieved through different axillary, infraclavicular or supraclavicular approaches. Infraclavicular brachial plexus block is widely used for anesthesia/analgesia of the distal upper extremity up to the shoulder, has gained popularity in recent years especially with the use of ultrasound in regional anesthesia. Several infraclavicular approaches have been proposed to improve the quality and reduce the risk of complications, from the approaches described in the infraclavicular fossa: the vertical, coracoid, lateral and lateral sagittal approach.
Objective: To determine if the costoclavicular block which a recently described approach is as effective as the ultrasound-guided coracoid block according to the literature.
Methodology: Review of the bibliography of scientific articles resulting from research, review, case reports, among others in the pages PubMed (www.pubmed.com), electronic database OVID National Institute of Rehabilitation LGII in the journals Anesthesiology, Anesthesia & Analgesia, Regional anesthesia, Regional Anesthesia & Pain Medicine limited to the period of time between January 1, 2012 and October 15, 2017, using as keywords claves «infraclavicular brachial plexus block», «brachial plexus block via infraclavicular coracoid approach» y «brachial plexus approach costoclavicular approach».
Conclusion: After the review of the literature and the comparison between both techniques of infraclavicular brachial plexus block, it is concluded that the approach of the costoclavicular ultrasound-guided block is anatomically feasible and with high clinical efficacy reaching the point of not having a statistically significant difference in relation to the coracoid approach which makes it an effective option within the procedures of upper limb.
