Transcubital access in cardiac catheterization: Initial experience of a radialist center

ABSTRACT
Introduction: When the radial route is not possible during coronary catheterization, transcubital access could be an attractive option.Objetives: To evaluate the use of this access and establish its effectiveness with respect to transfemoral access in diagnostic and therapeutic procedures.
Methods: A retrospective cohort study was conducted between January 2014 and June 2016 at the "Hermanos Ameijeiras" Clinical Surgical Hospital.
Results: The cubital access group consisted of 55 patients and 486 of femoral access. The ulnar route was used more frequently in elective procedures (70.9% vs 37.9%, p = 0.01). During angioplasty, the femoral artery was associated with longer fluoroscopy times (13.2 ± 10.0 vs 8.4 ± 5.3, p = 0.02). The success rate in the ulnar group was 100% and 95.5% for coronary angiography and angioplasty respectively and at 30 days of follow-up the rate of major adverse events was 1.8% vs 6.4% in the femoral route (p = 0.23).
Conclusions: En comparación con el acceso femoral el abordaje transulnar resulta una alternativa efectiva y segura tanto para el cateterismo diagnóstico como terapéutico.
