Spontaneous pneumomediastinum showed by cervical emphysema, pneumo-rachis and pneumoperitoneum: report of three cases

ABSTRACT
Spontaneous pneumomediastinum (SPM) is an uncommon disorder that usually occurs in young people without an apparent precipitating factor or disease. The term spontaneous talks about to an absence of relation with trauma or other identifiable pathological processes. The pathophysiologic mechanism responsible is alveolar rupture in pulmonary interstitium. Dissection of gas towards hilium and mediastinum is produced by an episode of acute high intrathoracic pressure. The usual mechanisms that produce pneumomediastinum include severe acute asthma, toxic induced bronchial hiperreactivity, and barotrauma caused by Valsalva maneuver, common history during marijuana smoking. In this study, three cases of SPM are presented, one was due to marijuana smoking, and other two were due to bronquial hiperreactivity secondary to infections of upper respiratory tract, as reported in literature, with conservative treatment and no future consequences.
