Osteogenic distraction for three-dimensional reconstruction of the atrophic alveolar ridge in the anterior maxillary segment

ABSTRACT
Reconstruction of the alveolar ridge through osteogenic distraction may be indicated for the atrophic alveolar process resulting from maxillofacial trauma, periodontal diseases and pathological or congenital deformities. Therefore, alveolar distraction osteogenesis is another strategy to obtain a favorable alveolar shape for rehabilitation with dental implants, aiming to gain height, anteroposterior length, and sometimes width. The principle of osteogenic distraction, derived from the orthopaedist Ilizarov, has been demonstrated for alveolar modification. The alveolar bone can be distracted in all directions to gain height and thickness. The osteogenic distraction to reconstruct the poor alveolar crest is a predictable technique that allows the regeneration of soft and hard tissues, to place implants in an ideal situation that will satisfy the current prosthetic rehabilitation, both aesthetically and functionally. However the treatment plan should be a combination of disciplines: prosthesis, orthodontics and surgery.
