Actualities in handling of sudden sensorineural hypoacusia

ABSTRACT
Since the sudden sensorineural hearing loss (SSNHL) was described for the first time by McCabe (1979), many have tried to define, explain and handle this upheaval correctly. The SSNHL is considered an otologic emergency that requires immediate intervention. In agreement with recent reports, USA has an annual incidence of SSNHL of 5- 20 cases by each 100 000 inhabitants. Although the diagnosis of this entity is not complicated, it is difficult to establish an effective and standardized handling because its etiology is not known with certainty. By such reason, the present study was made in order to review the actual treatment, the recent studies and the new modalities of handling of this pathology. English literature of 1997 to 2004 was reviewed, in search of relevant studies, controlled, randomized and prospective, in humans and animal models, with special approach in physiopathology and treatment advances. Recent results in use of antivirals, systemic and intratympanic steroids, alone and combined, and the safety of its use, vasodilator hemorreaologic, immunosuppresors, prostanoids, magnesium, carbogen, relation between HLA and SSNHL and prognosis, association with serum antiendothelial cell antibodies were examined. Conclusions are that the results are promising with individualized therapeutic options, that are letting to be empiric, even though there is the need of a consense in management of this pathology.
