Office laryngeal chromoendoscopy in patients with recurrent respiratory papillomatosis

ABSTRACT
Background: Recurrent respiratory papillomatosis (RRP) is a viral disease that causes epithelial exofitic lesions mainly in larynx and trachea with two peaks of presentation, between 2 to 5 years of age and the second to third decades of life. Diagnosis is through flexible laryngoscopy; however, it is not possible the visualization of all the lesions present, so in 2014 the chromoendoscopy was proposed to improve the visualization of lesions with the application of dyes in the mucosa.Objective: To report the usefulness of chromoendoscopy as a diagnostic and staging method in patients of the Laryngology Clinic in the National Medical Center 20 de Noviembre, Mexico City.
Material and Method: A quasi-experimental study was done from September to November 2015 in over 18 year-old patients with RRP diagnosis, performing in the office a 70º laryngoscopy with a three-chip camera before and after the application of the indigo carmine dye. The videos obtained were randomized and evaluated by 2 researchers, and the collection and comparison of results by a third researcher. The test of Shapiro-Wilk, Student’s t-test and Kappa coefficient were applied.
Results: A total of 24 videos were captured; in relation to observer 1, at comparing grades obtained with conventional laryngoscopy and chromoendoscopy it was found an increased anatomical grade of Derkay index (t = -2.966, p ‹ 0.05) obtained with chromoendoscopy. Observer 2 also found an increased anatomical grade of Derkay index (t = -2.549, p ‹ 0.05) of chromoendoscopy compared to conventional laryngoscopy.
Conclusions: It is demonstrated the superiority of laryngeal chromoendoscopy for the visualization of lesions and the usefulness of the application of Derkay staging system.
