Valvular Replacement Surgery in people older than 75 years

ABSTRACT
Introduction: In Cuba persists an increased rhythm of aging of the population because of low fertility, what turns us into one of the countries more aged of America. This brings an increase of the own affections of the aging along, and between this one’s predominate the heart valve diseases. Objectives: To know mortality and complications in the 30 days after the valve surgery in patient elders of 75 years. Methods: We accomplished a documentary, observational, descriptive, retrospective Investigation, where we analyzed the patients operated belonging to cardiac surgery in the Instituto de Cardiología y Cirugía Cardiovascular (ICCCV), between January 1978 and May 2016 (1 585 cases of heart valve surgery in 36 years). The sample was made of 39 patients from 75 years with heart valve surgery in the analyzed period. Results: The quantity of surgeries in elders of 75 years is inferior to the one of international reports. The more accomplished surgery is the valve aortic substitution. Mortality is paradoxically inferior to the younger patients. The renal dysfunction and the perioperative myocardial infarction (MI), happens with twice the amount of frequency between the older population of 75 and on. Discussion: The indication of the surgery in the elder is complex and should have in account the disparity between the biological and chronological age, the concomitant diseases, the autonomy and the familiar support. Conclusions: Advanced age is not an isolated opinion to advice against the cardiac surgery in the old patient. The decrease of the perioperative morbi- mortality will depend fundamentally on the correct selection of the patients.
