Intracranial broken aneurysm: surgical management vs. endovascular coil

ABSTRACT
Objective. This is a retrospective study whose aim is to compare the surviving years obtained as a result of the treatment by the surgical and endovascular viae to manage the SAH.
Method. By a follow-up revision of the patients who have undergone surgery and endovascular treatment, a retrospective study is carried out so that we can compare the surviving years’ indices in both experimental groups in the period from January 1999 to January 2003, in the Central Military Hospital, and considering the clinical stage according to Hunt and Hess’ scale, at their income.
Results. The results for both groups managed either by surgery or endovascular treatment did not make any significant statistical difference (p = 0.39) as to the patients in clinical stage of classification Hunt and Hess I and II. Hunt and Hess’ stages III-V (p = 0.005), and the lengthness of the affected area by the aneurysmal rupture (p = 0.032) were poor forecast factors; this group was only surgically treated due to the specific therapeutical indications.
Conclusions. The endovascular treatment for the acute rupture of intracranial aneurysms in patients with hemorrhages Hunt and Hess grade I and II present clinical results similar to those obtained in early clip-surgery for broken aneurysms. As to the endovascular treatment, it is the choice management for patients with hemorrhage Hunt and Hess grade I and II.
