Medulloblastoma with leptomeningeal carcinomatosis

ABSTRACT
Medulloblastoma is a relatively common tumor in children, accounting
for 20% of primary tumors of the central nervous system in children;
it has a tendency to spread along the neuroaxis through spinal fluid
at the time of diagnosis. Metastasis is associated with poor prognosis
and has mean survival of three to six months. Metastasis has been
observed predominantly in the brainstem and only 0.6% affects the
spinal cord. We present the case of a patient of 5 years of age who
presented with progressive cephalea and ataxia of one month’s evolution;
It was decided to take a single-phase computerized tomography
of the skull, which found dilation of the ventricular system with tran sependymal migration of spinal fluid, for which reason the patient
underwent emergency surgery to implant a peritoneal ventricular
bypass valve. The patient was admitted to our unit to continue the study
protocol, with magnetic resonance of encephalon and neuroaxis, with
administration of gadolinium, identifying a space-occupying lesion
in the cerebellum and leptomeningeal metastasis. The diagnosis was
confirmed by medullary biopsy.
