Serum prealbumin in the recognition of hospital manutrition

ABSTRACT
Rationale: Prealbumin (PAB) is a hepatic
secretory protein used in the nutritional
assessment exercises due to its shortened halflife
and ease of analytical determination. PAB
might identify those malnourished patients at
risk of complications and death after elective
surgery.
Objective: To assess the behavior of
PAB as marker of hospital malnutrition.
Study design: Prospective, longitudinal, analytical.
Study serie: Eighty-seven patients (Males:
55.1%; Ages ≥ 60 years: 41.3%; Length of stay:
14.9 ± 22.1 days; Mortality rate: 14.9%)
assisted at the “Hermanos Ameijeiras” Clinical
Surgical Hospital (Havana Citym, Cuba)
between September 2014 and December 2015
(both included); and were subjected to
nutritional intervention.
Material and method: Nature and strength of associations between serum PAB, on one hand, and hospital length of stay and mortality, on the other; were assessed.
Resultados: Average PAB on admission was 0.11 ± 0.07 g.L
-1. Eighty-two-point-seven percent of patients presented with PAB ‹ 0.2 g.L
-1. Basal
PAB values were independent from Body Mass Index (BMI) and score assigned by means of the
Subjective Global Assessment (SGA). PAB was also independent from hospital length of stay
and mortality. Basal PAB values increased 3 and 7 days after nutritional intervention. Changes in
PAB might be explained given the reduction of systemic inflammatory response and hypercatabolism.
Conclusiones: Decreased PAB might distinguish those patients signaled for nutritional intervention.
