Reconstruction of the head and neck with vertical trapezius myocutaneous flap

ABSTRACT
Microsurgical free flap has gained the popularity over pedicle flap nowadays in the reconstruction of head and neck. However, pedicled flaps and especially myocutaneous still remain a promising alternative and have a remarkable position in selected patients. The vertical portion trapezius musculocutaneous flap is used to cover defects in back and lateral neck regions, scalp and skull cap. This is a thin and pliable flap with a very long constant pedicle, a wide arc of rotation and minor donor-site morbidity, permitting a safe flap elevation. We present a case of the use of this flap to reconstruct an occipital wound, its design and details of the reconstruction. We recommend this versatile myocutaneous flap as an alternative to microvascular free tissue transfer for the reconstruction of defects in the head and neck region.
