Safe and effective combination tropicamide and phenylephrine for mydriasis in preterm neonates.

ABSTRACT
Purpose: To determine security and effectiveness as a mydriatic drug of the combination of tropicamide 0.8% and phenylephrine 5% in newly born premature patients.Materials and methods: A prospective study was made in 24 newly born premature with weight smaller than 1500 grams and less than 34 weeks of gestation with risk of developing retinopathy of prematurity. Tropicamide 0.8% was administrated in both eyes combined with phenylephrine to 5% in two doses, with interval of 15 minutes in prematures between the third and fourth weeks of life. Arterial pressure was monitored (PA), heart frequency (FC), oxygen saturation (Sat O2), before the administration of the drops and 10, 25, 40 and 50 minutes later. Three readings were made for each measure and an average was taken out. The pupil diameter was measured in millimeters one hour before and one hour later to evaluate the ocular fundus. Results: A satisfactory mydriasis average of 6.1 mm of pupilar diameter was achieved. Significant changes of the arterial pressure were not presented neither of the heart frequency neither of the oxygen saturation. Decelerations of the cardiac frequency were observed in isolated cases until 8 pulsations per minute without being statistically significant. Conclusions: The combination of tropicamide 0.8% and phenylephrine 5% is reliable and effective for mydriasis in newly born premature patients. In spite of the results all the premature patients should always be monitored during the fundus examination.
