Cutaneus metastases due to a renal cell carcinoma

ABSTRACT
Introduction: renal adenocarcinoma (hypernephroma or Grawitz tumor) is the most frequent tumor in adults; it represents 90-95% of all kidney tumors and 2-3% of all human cancers.Objective: to present a case of renal adenocarcinoma with initial extra-renal manifestations in the form of multiple cutaneous metastases, of interest because this is a rare form of presentation of the disease.
Case presentation: a 70 year-old patient, retired, with no personal or family history of interest; for six months he has presented lesions on the skin of the anterior wall of the trunk and fever of 38-39 °C. The physical examination revealed that the patient presented malnutrition, asthenia, anorexia, nodular and erythematous skin lesions that were discretely pediculated, pulsatile, nonpainful to pressure, well delimited and non-ulcerated. The abdominal ultrasound showed a solid mass in the upper pole of the kidney. Exeresis was performed, and the results of the immunohistochemical and anatomopathological tests showed the presence of positive vimentin and cytokeratin, as well as the histological compatibility of the lesion with metastases of renal cell carcinoma. Nephrectomy of the right kidney was performed; the anatomopathological study of the sample confirmed that it was a renal clear-cell carcinoma.
Conclusions: it should be taken into account that the appearance of cutaneous metastatic lesions as an initial presentation of renal adenocarcinoma is very rare, so the presence of these lesions should be a warning sign for specialists.
