Hand assisted laparoscopic nephrectomy and three ports on a pyelonephritis xantogranulomatous kidney: a case report

ABSTRACT

Background. Xantogranulomatous pyelonephritis is a rare, chronic and severe entity. Hand assisted laparoscopic nephrectomy is an effective and safe procedure. Objective. To demonstrate the surgical technique of a laparoscopic nephrectomy with the harmonic scalpel, the hand-assist device, and three ports, on a large xantogranulomatous kidney. We present the case description and literature review. Material and methods. We present the case of a female patient with xantogranulomatous pyelonephritis that underwent laparoscopic hand-assisted nephrectomy. Results. The procedure was uneventful and the patient had a favorable postoperative course with no complications. She was discharged 48 hrs after the procedure. Discussion. Laparoscopic hand-assisted nephrectomy is a safe and effective procedure. Xantogranulomatous pyelonephritis is associated with infection, obstruction and lithiasis. Conclusion. This procedure reduces postoperative pain and hospital stay.
