ABSTRACT

Objective. To present a 70 year-old patient with an infrarenal abdominal aortic aneurysm, biltateral large common iliac aneurysms without proximal or distal landing zone and bilateral hypogastric aneurysms who was treated with a conventional bifurcated aortic endoprosthesis with preservation of the left hypogastric artery with an aorto-hypogastricendoconduit made with covered Stents.
Background. Bilateral iliac aneurysms may require sophisticated and complex reconstructions. Solutions must be sought if there is no landing zone to avoid bilateral hypogastricembolization.
Material and methods. An EVAR procedure was done with a bifurcated aortic endoprosthesis preserving the hypogastric artery with aorto-hypogastricendoconduit.
Results. The procedure was done without complications, with good postoperative result with the big advantage of avoiding the known inconvenients and risks of bilateral hypogastricembolization.
Conclusions. The repair of bilateral common iliac and hypogastric aneurysms with an endoconduit is a useful tool to avoid bilateral hypogastricembolization and further complications from this procedure. It is technically feasible but it needs endovascular dexterity.
