Clinical manifestations and etiologies related to febrile seizures

ABSTRACT

Introduction: febrile seizures are one of the most common causes of admission to emergency services and of appointments with the neuropediatricians.
Objective: to describe the clinical manifestations and the etiology related to first febrile seizure on hospital discharge.
Methods: retrospective and observational study of 327 children seen at the emergency service of Centro Habana pediatric hospital from January 2005 to December 2013. The sample included 313 patients (185 girls and 128 boys) diagnosed with first complex or simple febrile seizures whereas 14 children with other paroxistic events were excluded. The children were distributed into three age groups and the age of the first febrile seizure was compared with the clinical diagnosis on hospital discharge. Symptoms, clinical signs and analytical exams on admission to the service were all registered.
Results: one-four years old group and males predominated (74.1 %). The simple febrile seizures prevailed in 85,9 % of cases and in older than one year-old children with 69 % (p ‹ 0.05). The febrile seizures appeared in the first 24 hours of fever in 77.2 % of children (20.1 % in the first hour). In 89 children (28.4 %), fever was the only symptom or initial sign related to the first febrile seizure; 174 (55.6 %) showed symptoms and signs in the upper respiratory tract whereas the most common infections occurred in this tract (62.2 %). Viral etiologies (68.6 %) prevailed over the bacterial ones (27 %). Cerebrospinal fluid tests were performed in all the children with no evidence of infection in the central nervous system.
Conclusions: simple febrile seizures related to viral infections are the most common ones.
