Collagenous gastritis. A pediatric case report

ABSTRACT

Collagenous gastritis is a rare disorder characterized by the presence of a thickened subepithelial collagen band associated with entrapped dilated capillaries and inflammatory cell infiltrate in the lamina propria. Very few pediatric cases have been reported. Most authors describe two clinicopathological groups: the first one in children, with symptoms of upper gastrointestinal tract, and the second one in adults, usually associated with autoimmune disorders. An eight-year-old girl presented with epigastric and chest pain refractory to treatment with proton pump inhibitors, antacids, and diet. Endoscopy was performed, in which a nodular appearance of the gastric mucosa was found. The corresponding antral biopsies showed irregular thickening of the basement membrane of the surface epithelium (› 10 µm), with entrapment of vascular and inflammatory cell structures. An effective treatment for collagenous gastritis in adults or children remains poorly defined. Both anti-inflammatory and anti-secretory measures and a gluten-free diet have been tested. Most of the reported cases followed a benign course.
