Bile duct hamartoma. Case report

ABSTRACT
Introduction: bile duct hamartoma is a rare liver injury, consisting of a deformation and disruption of the bile ducts, which form cystic structures of variable size. It is significant that the development of bile duct hamartoma can cause diagnostic confusion with liver metastases and hepatocellular carcinoma.Objective: to present the case of a patient with bile duct hamartoma because this being a rare entity, liable to cause diagnostic confusion, and not appear any case described in cuban literature reviewed.
Case presentation: a male patient of 46 years old, white, with a history of hypertension and treatment with enalapril, who presented a symptom picture compatible with acute cholecystitis; complementary tests showed normal values. Open cholecystectomy was performed and, in the anterior hepatic segment five, was identified a subcapsular whitish nodular lesion of 2 cm; excisional biopsy of the lesion for histopathologic examination was carried out. The diagnosis was bile duct hamartoma.
Conclusions: it is considered prudent, given the diagnostic uncertainty between benign hepatic lesions, primary hepatocellular carcinoma and metastatic liver disease, perform liver biopsy for histopathological examination because this is a direct way to diagnose biliary hamartoma and classify the degrees of injuries caused by it.
