Disseminated histoplasmosis in two patients with HIV

ABSTRACT

Acquired immunodeficiency syndrome (AIDS) is associated to opportunistic infections caused by Mycobacterium tuberculosis, Histoplasma capsulatum var capsulatum and other fungal, bacterial and viral agents. Disseminated histoplasmosis has been recorded in one of every 5,000 to 50,000 infected people, specially immunosuppressed patients and small children. Since initial description of H. capsulatum in 1934, there have been few published cases of H. capsulatum yeasts demonstrated in the peripheral blood smear. This paper presents two clinical cases of disseminated histoplasmosis. The fungus was stained in the cytoplasm of the polymorphonuclear leukocytes, as well as into the monocytes of the peripheral blood smears. Clinical manifestations, diagnostic laboratory methodology and advances in the chemotherapy of histoplasmosis are described with a review of disseminated histoplasmosis selected publications. A careful search of H. capsulatum in peripheral blood and bone marrow smears seems to be warranted in examining febril AIDS-patients.
