Propofol and remifentanyl for tracheal intubation without neuromuscular relaxants with video-laryngoscope

ABSTRACT
Background: Since the advent of short-acting opioids, tracheal intubation was possible when these drugs are used in combination with propofol. In situations where the use of neuromuscular relaxants is unwanted, either by blocking contraindication to not have adequated monitoring or in cases where intubation is necessary but not neuromuscular relaxation; tracheal intubation can be achieved without neuromuscular relaxants when combinations of remifentanyl and propofol are used. The main objective of the study is to determine the conditions of intubation in patients undergoing anesthesia induction with propofol bolus plus infusion of remifentanyl.
Material and methods: 46 patients were randomized through a sealed system into two groups (group S = no relaxing; group C = with relaxing). The induced with propofol bolus 2.3 mg/kg, the content of the syringe, infusion of remifentanyl 0.4 µg/kg/min. An expert anesthesiologist proceeded to intubate the patient through video laryngoscope.
Results: 72.72% of patients who received neuromuscular relaxants showed excellent tracheal intubation conditions, while 27.27% had good condition.
Conclusions: A bolus of propofol 2.3 mg/kg with an infusion of remifentanyl 0.4 µg/kg/min allow to obtain intubating conditions with video laryngoscope in 100% of cases.
