Is the Pavlik harness an effective treatment for femoral shaft fractures after obstetric trauma in neonates?

ABSTRACT
Background: Femur fractures after obstetric trauma are rare, the use of the Pavlik harness has been described for infants; however, previous reports have included only short-term follow-up and no study has focused specifically on obstetric trauma as a cause. This study describes the treatment results of three patients with obstetric fractures of the femur treated with a Pavlik harness.
Method: The clinical and radiographic records were reviewed for three babies treated with a Pavlik harness for an isolated femoral shaft fracture occurred at birth. Clinical and radiographic follow-up was performed at a minimum of two years. Complications were noted and angular deformity and limb length discrepancy evaluated.
Results: Three patients were included, mean time in the harness was 3.2 weeks. Average age at follow-up was 4 years. No significant angular deformity or limb length discrepancy was noted and no complications occurred.
Conclusion: Our study supports the evidence that angulation will remodel and the Pavlik harness is an effective method of treating femoral shaft fractures in neonates after obstetric trauma with a significantly lower cost and complication rate compared to other treatment modalities.
