Upper gastrointestinal bleeding and hepatic hydrothorax as an unusual manifestation of an overlap syndrome with systemic lupus erythematosus and autoimmune hepatitis

ABSTRACT

Hepatitis in patients with systemic lupus erythematosus is uncommon; most cases are related to drug toxicity, infections and, less frequently, autoimmune or related to systemic lupus erythematosus itself. We report the case of a 66-year-old woman with a previous diagnosis of autoimmune hepatitis admitted for an episode of non-variceal upper gastrointestinal bleeding and right hydrothorax; during her hospital admission, she developed clinical features consistent with systemic lupus erythematosus involving the central nervous system, hematologic, articular, and positive antinuclear antibodies not related to autoimmune hepatitis. The overlap between systemic lupus erythematosus and autoimmune hepatitis is rare (2.7%). The diagnosis represents a clinical challenge and often requires a multidisciplinary approach since both conditions may have different clinical courses
