ABSTRACT

Chest trauma is common in trauma patients and is a significant source of disability, morbidity and mortality. Flail chest is the most severe form of blunt injury with a mortality rate of 10 – 20% typically accompanied by pulmonary contusion, which is the most common injury in blunt trauma, occurring between 30 – 75% of cases . The survival of these patients depends primarily on how fast they are transferred to a specialized trauma center for their resuscitation and stabilization, as well as to the management scheme that is chosen while they are hospitalized. Despite the prevalence and recognition of flail chest associated to pulmonary contusion, the morbidity and mortality rates of this compound pathology remain constant without showing improvement in the last three decades due to poor understanding of its complex pathophysiology and the lack of appropriate management guidelines.
