ABSTRACT
The always worrisome balance between thrombosis and bleeding in the patient under the effect of an anticoagulant increases by the need of an invasive procedure, resulting in complex algorithms analyzed in this written in order to propose practical protocols that facilitate us to acquire reasoned knowledge of the perioperative management of the patient receiving anticoagulant therapy. If surgery is needed on a patient under anticoagulant therapy the following steps are suggested: establishing a real risk of thrombosis based on the procedure to be performed and hypercoagulable patient characteristics, establishing the risk of bleeding in relation to the procedure and the anticoagulant given to the patient, knowing the pharmacokinetics of anticoagulant in relation to the degree of renal function, establishing the need for a “bridge therapy” and its exact start and retreat in relation to certain variables and postoperative resumption of oral anticoagulant. Every one of the above steps is discussed in this article.REFERENCES
