ABSTRACT
Spontaneous perforation of the
esophagus: that’s the definition
of Boerhaave´s syndrome.
Others call it effort rupture
of the esophagus. This deadly
acute disease most commonly
results from a sudden insult
of intraesophageal versus
intrathoracic pressures caused
by straining or vomiting that
produces shearing forces of
this digestive organ, leading
to complications that, without
treatment, will become fatal.
There are other causes of
esophageal rupture that
are far more common than
Boerhaave´s syndrome that
will not be addressed in
this article, like ingestion of
substances and iatrogenic
perforation. However, being
rare is not the same as being
benign. Boerhaave´s syndrome
management is far more
complex than other obvious
esophageal perforations, since
it is spontaneous and clinically
subtle in most cases. The
diagnosis can be done with
contrast imaging studies. Its
treatment is not always surgical,
and is based upon preventing
further complications.
