Cricopharyngeal surgical myotomy

ABSTRACT
Cricopharyngeal dysfunction is produced when muscle is not relaxed
properly during swallowing, due to delayed or incomplete relaxation of
cricopharyngeal muscle that may be due to multiple causes. Common
symptoms are dysphagia (98%), odynophagia, breathless sensation,
halitosis, cough and nasal regurgitation. Aspiration and weight loss
occur in severe cases, malnutrition appears even in 30% of patients
and disorders of swallowing safety (penetrations and aspirations) occur
in two thirds of patients, with a high risk of respiratory infections and
aspiration neumonia. Treatment of cricopharyngeal muscle dysfunction
includes swallowing therapy, dilatation, botulin toxin injection, transcervical
cricopharyngeal myotomy and endoscopic surgery with laser.
Cricopharyngeal myotomy is the standard surgical intervention used as
treatment of swallowing disorders originated by an isolated dysfunction
of cricopharyngeal muscle.
