Efficacy of specialized nutritional therapy in patients submitted to moderate metabolic stress

ABSTRACT
Background: The specialized nutrition therapy is a useful therapeutic tool as first line treatment in metabolically stressed patients.Objective: To determine the efficacy of nutritional therapy in patients with moderate specialized metabolic stress admitted in the area of internal medicine at a public hospital in Mexico.
Material and Method: A nonrandomized pilot study was conducted with a sample of 10 patients divided in: Nutritional Support (NS) and Specialized Nutrition Therapy (SNT), non-randomly allocated according to medical and bioethical considerations. Patients were nutritionally assessed by Subjective Global Assessment (SGA), determination of serum albumin, total protein, serum electrolytes and total lymphocyte recount in monitoring every seven days during their hospital stay, evaluating albumin, total protein, lymphocytes, nutritional diagnosis and clinical progress. Group 1 (NS) was handled only with polymer formula and standard hospital diet; group 2 (SNT) treated with enteral formulas and/or individualized parenteral supplemented with pharmaconutrients and/or immunonutrients; data were processed with SPSS 17 package for obtain measures of central tendency and analysis with Student t (independent data).
Results: Positive changes were observed in biochemical and anthropometric parameters of SNT group; lower mortality in SNT with NS ratio of 30%, 80% in SNT efficacy against 20% for the NS.
Conclusions: Although is not statistically significant due to the small study group, specialized nutritional therapy can be used as first-line treatment option for the metabolically stressed patient, for its low cost of operation and its overall clinical efficacy.
