Impact of the use of probiotics upon selected indicators of the nutritional status of the hospitalized patient

ABSTRACT
Rationale: The use of probiotics might be useful
for nutritional and clinical recovery of the
patient at risk of malnutrition and immune
depression as consequence of the response to the
medical-surgical treatment.
Study design: Quasi-experimental study for assessing the effectiveness as a bacterial interference therapy for the restoration of the intestinal ecosystem
and the nutritional status of the patient of a
yogurt incorporating selected, viable probiotics
(Therapy A)
vs. another one with a regular
bacterial composition (Therapy B).
Study location: “Luis Díaz Soto” Hospital (Havana
City, Cuba).
Study serie: Seventy-three patients
(Males: 58.9%; Ages ≥ 60 years: 46.6%)
assisted at the institution between June 2011
and June 2012 (both included) in whom systemic
sepsis, bacterial overgrowth and diarrhea
associated with prolonged antibiotic therapy,
and failure of healing causing anastomosis
dehiscence and surgical wound sepsis
concurred.
Material and method: Patients were
assigned randomly to one of each therapy:
Therapy A: 52.1%
vs. Therapy B: 47.9%.
Therapeutic effectiveness was measured from
change in anthropometric and biochemical
indicators of nutritional status after 7 days (at
least) of treatment with 750 milliliters of the
yogurt (250 milliliters every 8 hours). Additional
treatment cycles were prescribed according with
the patient´s response.
Results: Sixteen-pointnine
days of treatment per patient were
completed (Therapy A: 17.1 ± 12.9
vs. Therapy
B: 16.5 ± 13.1; p › 0.05). Daily energy intakes were independent from the assigned treatment:
Therapy A: 2,213.4 ± 751.1 Kcal.24 hours
-1 vs. Therapy B: 2,250.6 ± 705.4 Kcal.24 hours
-1 (t = -0.217; p › 0.05; test for comparing independent means). Adverse reaction rate (manifesting as diarrhea) was 2.7%. Microbial interference therapy only resulted in an increase of serum cholesterol basal values: Therapy A: +0.2 ± 0.9 mmol.L
-1 vs. Therapy B: -0.14 ± 0.9 mmol.L
-1 (p ‹ 0.05).
Conclusions: Microbial interference
therapy with probiotics provided as a yogurt can
cause increases in serum total cholesterol. If the
change observed in this nutritional indicator
represents a better prognosis for the patient
remains to be proved.
