Atherogenic index and its relationship with estradiol levels in pre- and post-menopausal women

ABSTRACT
Introduction: The incidence of cardiovascular disease after menopause may be partly caused by changes in the plasma lipid levels that occur following the menopausal transition.
Objective: To investigate the relationship of the menopausal status and its related hormonal variation of estradiol with plasma lipid concentrations and atherogenic index.
Material and methods: A retrospective, observational, cross-sectional study was performed, in which the Castelli index (total cholesterol/HDL cholesterol) and estradiol levels were calculated out of the lipid profile of 62 premenopausal and 93 postmenopausal patients who attended the IMIEM’s Hospital of Gynecology and Obstetrics from January to December 2013. Student’s T test and Pearson’s test of correlation were used for the statistical analysis. P-values of ‹ 0.05 were considered to be statistically significant.
Results: The correlation between the Castelli index and the levels of estradiol was negative and significant at p = 0.001; a significant increase was presented in the levels of triglycerides, LDL cholesterol and total cholesterol in postmenopausal women, with a significant decrease in the levels of HDL cholesterol. The atherogenic index was significantly increased in postmenopausal women compared to premenopausal.
Conclusions: Menopause leads to changes in the lipid profile by reducing HDL and elevating total cholesterol (TC), triglycerides (TG) and LDLcholesterol, thus increasing the risk for cardiovascular disease. These changes are caused by the reduced estrogen concentrations that are seen in menopause.
