ABSTRACT
Objective: Report the results obtained with the surgical management of mediastinal cysts in the Calderon Guardia Hospital.Methods: Following approval from the Ethics Committee, a retrospective analysis of medical records of patients treated in the Thoracic Surgery Department with mediastinal cysts between 1994 and 2014 was conducted.
Results: Mediastinal cysts were found to be more common in women with a gender ratio of 3.5: 1, and average age 43 years. Lesions were localized in the middle mediastinum in 9 cases, 7 in posterior mediastinum and 2 anterosuperior. Clinical presentation in 13 cases was an incidental finding. The most common symptoms were cough and dyspnea. One patient presented as an emergency with a ruptured cyst. Another patient was studied for Myasthenia Gravis and thymic cyst was found associated with lymphoma. In 12 cases the cyst was removed by thoracotomy and in 6 cases by VATS. In 16 the resection was complete and in 2 the cyst wall was attached to mediastinal structures, however there was no recurrence. No incidence of operative mortality or complication was reported.
Conclusion: Mediastinal cysts are uncommon. Although smaller ones can be managed conservatively, surgical management is indicated for larger ones or those presenting with compressive symptoms due to the possibility of complications, the most frequent being internal bleeding. Surgery may be minimally invasive and is associated with low risk of complications.
