Extradural postoperative analgesia in patients undergoing orthopedic surgery of the lower limb: plain ropivacaine plus fentanyl vs plain bupivacaine plus fentanyl

ABSTRACT
A clinical, comparative trial was carried out to demonstrate whether the extradural infusion of plain ropivacaine plus fentanyl had a superior efficacy than plain bupivacaine plus fentanyl in relieving postoperative pain in patients undergoing
orthopedic surgery of the lower extremity. One hundred and forty patients, ages 18 to 93 and physical status ASA 1, 2 and 3, were randomized to two groups: Group
1 (control group, n = 70), plain bupivacaine plus fentanyl, and Group 2 (study group, n = 70), plain ropivacaine plus fentanyl. Both groups received the drugs by extradural infusion using a mechanical pumping device. Postoperative analgesia was evaluated using the Visual Analogue Scale; it was considered "satisfactory" when the patient rated it 0 through 3, and "unsatisfactory" when it was rated 4 - 10 along the first 24 hours. There were no statistically significant
differences in demographic variables among both groups. Analgesia was considered "unsatisfactory" in 9 patients in Group 1 and in 4 patients in Group ". We found
out that combined ropivacaine plus fentanyl for extradural infusion have a similar efficacy to bupivacaine plus fentanyl in the control postoperative pain in patients undergoing orthopedic surgery of the lower extremity.
