Ocular manifestations of miliary tuberculosis. Considerations on five cases.

ABSTRACT
Miliary tuberculosis (TbM) presents as the hematogenous spread of Mycobacterium tuberculosis (Mt) to different organs from a primary lesion in which persistent Mt remained viable. Ophthalmic tuberculosis manifests in various forms in each eye structure and the most frequent are granulomatous uveitis and choroidal granulomas.
Objective: To identify ophthalmic lesions in HIV-negative patients with TbM.
Methods and material: All patients with TbM diagnosed by chest X-ray and Mt-positive Lowenstein-Jenssen cultures of urine and sputum were subjected to ophthalmologic examination.
Result: Twenty-eight patients were included, among which five (18%) presented ophthalmic lesions compatible with TbM, two presented multifocal choroiditis, one choroidal granuloma, one chorioretinal scars, and one presented peripheral vasculitis sequelae.
Conclusions: Diagnosis of ophthalmic tuberculous lesions is difficult, thus it is important to perform a routine ophthalmologic examination in patients with TbM to achieve an early detection.
