Lung cancer: Metastasis distribution according to histological type. Necropsy study.

ABSTRACT
Introduction: The main histological types of lung cancer (LC) are adenocarcinoma (ADCA), squamous cell carcinoma (SQCC), small cell carcinoma (SCC) and large cell carcinoma (LCC). Determination of metastasis distribution is required for treatment design and prognosis.
Objective: To describe the most frequent sites of metastases common in LC from necropsies according to the histological type, and to compare our results with those in the literature.
Material and methods: Necropsy protocols and slides of cases pathologically diagnosed as LC were analyzed from January 1, 1985 to December 31, 2000.
Results: 205 cases of LC were found; the most common histological types were ADCA in 42.2%, SQCC in 18%, SCC in 16.1%, and LCC 12%. Metastasis sites for ADCA were regional lymph nodes in 75%, contralateral lung in 38%, and adrenal glands in 31%. The sites most frequently affected by SQCC were regional lymph nodes in 59%, contralateral lung in 40%, and adrenal glands in 24%. In the case of SCC, the regional lymph nodes were affected in 90%, distal lymph nodes in 36%, and contralateral lung in 24%. In LCC, the most affected metastasis sites additionally to lymph nodes were liver in 54% and pleura in 46%. All the studied cases showed locoregional and distal metastases.
