ABSTRACT
Portal vein gas is an ominous radiological sign, which indicates a serious gastrointestinal problem in the majority of patients. The radiographic images should be distinguished from other images with extra intestinal gas. Many causes have been identified and the most frequent one in adult patients is bowel ischemia and mesenteric vascular insufficiency, while umbilical vein catheterization is the dominant cause in neonates. The presentation of patients is variable and the diagnosis of the underlying problem depends mainly on the radiological findings and clinical signs. Under the presence of signs and symptoms suggestive of sepsis or intra-abdominal problems prompt surgical intervention is indicated. The aim of this report is to present the case of a woman in the third decade of life with severe neurological disease, who arrived in septic shock and with abdominal distention, in whom radiological images of gas in the portal venous system were found, probably related to necrotizing enterocolitis. We also analyze the clinical importance of gas in the portal vein as a radiological finding.
