Waist circumference, size of visceral fat and metabolic disorders in morbid obesity

ABSTRACT
Rationale: Obesity has become a worldwide epidemic. Body weight excess affects one-third of the Cuban population. Anatomical distribution of the excess of body fat might serve to establish metabolic risk factors and to determine comorbidities.
Objective: To assess if waist circumference is associated with visceral fat and occurrence of metabolic disorders in morbidly obese subjects (Body Mass Index ≥ 40 Kg.m
-2).
Material and method: Associations between size of visceral fat (measured by electric bioimpedance), waist circumference, peripheral utilization of glucose, fasting serum insulin, HOMA index, sera triglycerides and cholesterol were cross-sectionally assessed in 52 morbidly obese subjects (Women: 67.2%; Ages ≥ 60 years: 3.9%; High Blood Pressure: 76.9%; Diabetes mellitus: 7.7%) ambulatorily assisted at the “Hermanos Ameijeiras” Hospital (Havana City, Cuba).
Results: Seventy-one point one percent of the studied morbidly obese subjects showed ≥ 10 kilograms of visceral fat. Size of visceral fat was independent from waist circumference, BMI, the state of peripheral utilization of glucose, presence of insulin resistance, and sera lipid fractions concentrations. Likewise, size of visceral fat was independent from the presence of the Metabolic Syndrome.
Conclusions: In morbid obesity, occurrence of disorders in the peripheral utilization of glucose, presence of insulin resistance, state of sera lipid fractions and existence of Metabolic Syndrome are independent from the size of visceral fat. Others topographical locations of body fat (such as intra-parenchymatous fat) might have a higher impact upon body energy metabolism.
