Conservative management of perforations and leakage of the intrathoracic esophagus

ABSTRACT
Introduction: Esophageal injury caused by surgery, has a morbi-mortality of 10 to 12%. Because of its characteristics and behavior in the thorax it increases the risk of severe complications as mediastinitis and sepsis.
Materials and methods: A prospective, transverse, clinical and analytic study from January 1999 to January 2002 was done in patients with thoracic esophageal injury. Injury mechanism, site of the injury, radiologic, endoscopoic studies and the actual status of the patients were analyzed.
Results: There were six females, ages 2 to 6 years. Four with inferior esophageal injury and two with superior esophagus injury. Two were caused by esndoscopy; two, by Nissen’s procedure and two after esophageal resection and anastomosis for repair of esophageal stenosis (anastomotic leakage). Pleural effusion decreased on the tenth day; in one case it persisted for 21 days.
Discussion: The six patients had mediastinum enlargement; there was no pericardium or pleural purulent effusion. No patient required surgery, including the one with blood culture positive to Staphyloccocus, since no clinical or laboratory data of sepsis were present.
