Choledochal cyst: presentation, diagnosis and treatment

ABSTRACT
Background: Choledochal cysts are considered benign, although associated with malignancy, cholangitis, pancreatitis, and cholelithiasis. New technologies in imaging and surgery have generated controversy in the recommended diagnostic technique and management.
Objective: We conducted an evidence-based review of the world literature from the last five years to provide a unified management approach.
Material and methods: A search of electronic databases (Medline, Ovid y Elsevier ClinicalKey) using the term “choledochal cyst”, including all the articles published in English between January 2009 and August 2014. They were classified according to the main focus (epidemiology, classification, presentation, diagnosis, pathology, treatment, and prognosis). Information was compared and reported for each article in an objective manner.
Results: 64 articles were found. According to their main focus: presentation (4), diagnosis (12), pathology (11), treatment (28), and prognosis (9); they all described similar epidemiology.
Conclusions Choledochal cysts are a rare entity that requires resection to prevent complications. Hepaticoduodenostomy is associated with a higher incidence of complications. Laparoscopic resection is safe in expert hands. More studies with robotic resection in pediatric population are needed. Resection does not reduce the risk of malignancy to base level, so follow-up should be long term.
