Tuberculous empyema

ABSTRACT
Tuberculous empyema is a rare clinical disease, which frequently appears as a complication of pleural tuberculosis. However, certain clinical conditions can favor its development, for example, plombage, oleothorax, therapeutic pneumothorax. It can also evolve from a scar a pneumonectomy or from thoracoplasty. Not much information exists about the pathophysiology of the disease, and, unlike pleural tuberculosis, pleural empyema is produced by infection in the pleural space. The clinical and radiographic features are not different from pleural tuberculosis, but the presence of a fistula can complicate the clinical spectrum. Treatment requires antituberculous drugs and surgery.
The surgical approach is based on the drainage of the purulent liquid and may be as simple as a chest tube insertion or as aggressive as open pleurectomy or thoracotomy.
