Ethical medical and legal dilemma of the blood products transfusion in Jehovah witness

ABSTRACT
Introduction: Every time it is more frequent in the medical daily practice to find a Jehovah witness that rejects the transfusion of any blood products, independently of the risk that this produces for life.Objective: To offer the medical, ethical and legal foundations that sustain the dilemma of the blood products transfusion in Jehovah witness patients, as well as the design of a measuring system in order to minimize this conflict.
Method: The investigation went through two moments: descriptive- traverse and the elaboration of the proposal. An intentional sample of 28 specialists was carried out, for the higher frequency of facing this problem and by possessing a high professional, ethical and scientific prestige.
Results: There is insufficient knowledge on the conduct to assume with these patients in emergency situations, the use of other therapeutic alternatives and the results of free investigations or blood thrifty, the motivation and preparation of the professionals in order to respect the abstention to the transfusion should be increase, as hard as possible, in the curriculum of students and residents.
Conclusions: The measuring system seeks to avoid, diminish or postpone the use of blood products, on the base of the individualized use starting from clinical and sanguine parameters, appropriate preoperative preparation, hemostasis and use of several alternatives at the same time. It is recommended to disclose these results and validate them in hospitals.
