Prediction of mannan antigen of Candida in serum by means monoclonal antibodies for acute disseminated candidiasis diagnosis

ABSTRACT
Accute disseminated candidiasis is frequently observed in patients with immune compromise or important risk factors. Objectives. Using as a gold standard blood culture, the objective is to test the specificity, sensibility and prediction rates for the mannan antigen blood test. Methods: A retrospective, comparative, transversal study was made in a paediatric group of patients with accute disseminated candidiasis clinical diagnosis. From January 1rst 1996 to December 30th 1999. A total of 116 patients with suspected Candida infection were divided in two groups. Group A with confirmed diagnosis by blood culture and also the mannan antigen blood test, and Group B that included patients with clinical suspicion of acute disseminated candidiasis that was rule out with both; negative blood culture and negative mannan antigen blood test. Results: The results showed a sensibility of 72.7 % rate, specificity rate of 81.9%, positive prediction rate of 48.5% and a negative prediction rate of 92.7%. With a general accuracy of 80.1% rate for acute disseminated candidiasis diagnosis confirmation. Conclusions: The blood culture is still the most reliable diagnosis study for Candida infection. Also concluded that the mannan antigen detection blood test by means of monoclonal antibodies for acute disseminated candidiasis is a fast, non invasive, safe, and very reliable test due to its high specificity.REFERENCES
